Waterbury Health Access Program
Founded in 2003, the Waterbury Health Access Program (WHAP) combines the expertise of several clinical and non-clinical case managers and social workers to address access to care for low–income and uninsured people in Waterbury. The program is now embedded in Waterbury Hospital, Saint Mary’s Hospital, StayWell Health Center, Department of Mental Health and Addiction Services and Malta House of Care. WHAP also delivers lead case management for the Community Care Team. The WHAP program work aligns with the Access to Care health priority established by the Community Health Needs Assessment. The Community Care Team is a formalized clinical and community based organization integration project. The Community Care Team (CCT) is made up of over 40 social service and health care providers in the Greater Waterbury community that come together weekly to provide intensive case management and coordinate care for the highest utilizers of our hospital Emergency Departments and most vulnerable residents. The CCT uses the data platform, Unite Us, to efficiently and securely track and send referrals to Partners that are enrolled in the system. The unique care coordination database called Unite Us allows GWHP to track linkages and measure progress of patients with outcome indicators. The CCT works to link patients with the care they need and coordinate services for any social determinants of health or barriers they may be facing. CCT interventions tend to be long-term with the goal of helping communities of color that are housing challenged achieve stability and independence.

Be Well Bus
Our Be Well Bus is a free service to our patients. If you need a ride to and from the hospital we will pick you up.
For a ride call: (203) 573-7711.
The Be Well Bus runs Monday to Friday from 7 a.m. to 5:30 p.m.
Language Services
GWHN provides free aids and services to people with disabilities to communicate effectively with us, such as: qualified video sign language interpreter services, TTY, amplified telephone, personalized listening devices as well as free language services to people whose primary language is not English.

Addiction Treatment Open Access Program
- Open Access Admissions: For new patients seeking treatment for addiction, no appointment is necessary. Treatment begins with an initial comprehensive evaluation by our team of addiction experts. Admissions take place Monday through Friday at 8 a.m.

We accept most commercial insurances, as well as Medicare, Medicaid (Title XIX), and State Assistance (SAGA).

Youth Pipeline Initiative Programs
Since the Youth Pipeline Initiative was established in 2001 as a partnership between Waterbury Hospital and Waterbury Public Schools, its mission has remained the same: to close the achievement gap for minority and economically disadvantaged students in Waterbury so they can matriculate and compete nationally for placement in post-secondary education programs in preparation for health careers. We take pride in helping students realize their potential as they embark on their academic and personal journeys. To this end, during 2014 Waterbury Hospital provided over 1,000 students and parents in Greater Waterbury with unique educational programs that enhance the overall welfare of our community.
All programs are offered free-of-charge to participants.
