VNA Health at Home is a private home care agency that has provided professional healthcare services to patients in their homes since 1939. Located in Watertown, the agency serves a 16-town area including Beacon Falls, Bethlehem, Cheshire, Middlebury, Morris, Naugatuck, Oakville, Oxford, Plymouth, Prospect, Roxbury, Southbury, Terryville, Thomaston, Waterbury, Watertown, Wolcott, and Woodbury.
VNA Health at Home is a wholly owned affiliate of Waterbury HEALTH, a family of healthcare services in the Greater Waterbury region owned by Prospect Medical Holdings. As a member of Waterbury HEALTH, VNA Health at Home is able to provide coordinated, seamless care for patients making the transition from hospital to home.
Our vision and goal is to be the provider of choice for home healthcare in the Greater Waterbury area. Licensed by the State of Connecticut and Medicare-certified, VNA Health at Home adheres to rigorous standards of quality in support of our primary aim to provide the highest possible standard of care to our patients.

Our team of nurses, therapists, social workers, and home health aides use the latest technology available to provide quality and compassionate care in homes throughout the Greater Waterbury community. Referrals
VNA Health at Home accepts referrals by physicians, hospitals, nursing homes, patients, family members, and friends. Those eligible for home health care include individuals discharged from a hospital, skilled nursing facility, or rehabilitation facility but who are not fully recovered. They may have an illness, injury, recent surgery, or other condition that requires assistance with activities of daily living.

We request that you have the following information on hand before calling us in order to speed up the process:
- Demographic information: Name, birthdate, address, phone number, Social Security number, insurance details, emergency contact
- Physician’s name
- Basic reason for referral and services requested
- Current medications
We accept Medicare and Medicaid, along with most private insurance plans. Patients may privately for services not covered by insurance.
